General Info


All registration is done online. Once you successfully register online you will receive a confirmation email. Attached to the email you will receive the required medical forms that must be completed and uploaded through the site. Those forms and the instructions on how to upload to the site are located in the section below as well as the confirmation email. If you have not received your confirmation email, need special accommodations, or have any questions please contact Jose Robles-903-343-4182 (Roblesj@fau.edu).

Required Medical Forms

In accordance with Florida State law, Florida Atlantic University, and to ensure you child’s safety the following four documents are required to be completely filled out, signed, and uploaded to the site prior to the start of camp. Please click on each form below and follow the accompanying instructions. Once completed, follow the instructions below on how to upload the forms to the site.

The Medical Health Form must be completed and signed by a licensed medical practitioner. In lieu of the Medical Health Form you can include a physical check-up completed and signed by a licensed medical practitioner within the past year.

Authorization to Administer Medication (If this does not apply to your child you can skip to Section 4 and complete the information)

Acknowledgement of Responsibility Form (Print name, sign, and date)

FAU Parental Permission Form (Print name, sign, and date)

How to Upload Forms:
  1. Visit https://fausoccercamp.com
  2. Click the 'Log In Here' button at the top of the page
  3. Use the same log in information you used to initially register
  4. Click the 'Proceed to My Account' prompt or the 'Account' button at the top of the page
  5. Click the 'Files' button next to the order
  6. Click the 'Browse' button and select the file you wish to upload
  7. Click the 'Upload' button


Check-in will begin 30 minutes prior to the camp start time each morning.


All sessions are open and we encourage parents and coaches to attend.


Florida Atlantic University will have a Certified Athletic Trainer on site at all times.


All cancellations prior to 30 days before the first day of camp are eligible for a full refund (minus the service charge). Cancellations within 30 days of camp may receive a credit for a future camp or a refund minus a $75 service charge. There are no refunds or credits given for inclement weather. Camp credit is transferable to family members only.
